Overview

The Direct Care Specialist position works with individuals with disabilities in one of our residential program sites located throughout the Minnesota Iron Range communities, from Grand Rapids to Gilbert. Direct Care Specialists provide support and supervision to our clients by assisting them with daily living skills & personal cares, promoting community involvement and helping with household chores such as cooking and cleaning. Overview

The Community Support Specialist/Direct Care Specialist works 1:1 with semi-independent individuals with disabilities in their homes and local communities across the Minnesota Iron Range and surrounding communities. The position will support independent living and daily task management through teaching life skills, promoting community involvement and facilitating leisure activities. This position offers a high level of independent working, decision making and flexible schedule management based on client needs. Reliable transportation is needed to take clients on community outings (paid mileage reimbursement).

Overview

The Residential Services Supervisor (RSS) is responsible for the ground level operations in one of our residential program sites, which are located throughout the Iron Range. The RSS provides leadership and supervision to the direct care staff working in the assigned program, helps manage program and consumer finances, generates staff schedules, works with the Program Coordinator to develop and implement consumer goals, ensures a person-centered environment, and leads their team in an effective and positive manner to ensure consumer plans and goals are followed through. Overview

The Maintenance Technician will assist in the overall maintenance and repairs for facilities and program sites. Based on prior experience, Applicants may be considered for a level I or Level II Maintenance Technician; the level II positions will require higher levels of experience to support additional responsibilities and expectations.

Responsibilities

  • Supports the Mission, Vision, and Guiding Principles of NHS-Northstar.
  • Assist in providing maintenance repair and remodeling of company facilities, recommendations to support the Maintenance Manager and other Maintenance Technicians.
  • Assist in performing various carpentry, plumbing, electrical, and masonry duties.
  • Conduct yard and grounds maintenance such as mowing, trimming, and basic landscaping.
  • Remove snow and ice from drive and walkways using vehicles, plows and shovels.
  • Assist in vehicle maintenance and transporting company vehicles and trailers for oil changes and other repairs.
  • Conduct and assist in maintaining the household supply inventory and perform deliveries.
  • Assist as needed on the weekends on an on-call basis.

  • High School Diploma or GED is required
  • Post-Secondary education and training is desired but not required
  • At least six (6) months of experience in remodeling, construction, repair or maintenance is preferred

Responsibilities

  • Responsible for overseeing the quality of care delivered to clients, ensuring that it is carried out in an ethical and appropriate manner
  • Coordinates with physicians and other health care providers and ensures that clients receive the proper medical care determined by the treating health care professional
  • Oversees, under the direction of the Director of Residential Services, the administration of medications by unlicensed direct care staff
  • Provides on-going training and support to line staff and management in health related topics
  • Develop, implement, and monitor the health care plans/services to all clients as assigned and/or requested
  • Perform required individual clients assessments within license (RN or LPN) guidelines
  • Perform or arrange for medical treatments as prescribed
  • Provide and/or arrange for necessary training to agency staff (i.e. medication administration, CPR, First Aid, employee orientation, others as deemed necessary)
  • Understand and remain current with the health care needs of assigned clients
  • Provide necessary crisis intervention (medical in nature)
  • Participate in medical appointments with outside agencies
  • Monitor medication and health service practices to clients
  • Manage Psychotropic Medication Use Checklist (PMUC) on individuals with prescribed psychotropic medications
  • Prepare necessary health care reports and summaries for clients team meetings and outside agencies

Qualifications

Experience/Education/Training

  • High School Diploma
  • Graduation from an accredited school of nursing is required
  • Current Minnesota license as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) required
  • At least one (1) year of experience as a Licensed Practical Nurse or Registered Nurse
