Community Support Services

NHS Northstar provides Community Support Services under the DHS Home and Community Based Services Waiver Program.

Community Support Services are provided in St. Louis and Itasca County.

Community Support Services assists adults and children to live as independently as possible in their own homes. Our staff provide a variety of supports, geared toward all levels of abilities. We work with adults and children throughout Northeast Minnesota.

Individualized Home Supports with Training

Individualized Home Supports with Training is a teaching service for adults who live in their own home or their family’s home. This service is for individuals who need training, assistance, and supervision in at least one of the community living service categories:

  • Community participation
  • Health, safety, and wellness
  • Household management
  • Adaptive skills

These services are provided in a person’s home, community, or remotely.

Individualized Home Supports with Family Training

Individualized Home Supports with Family Training is a service that is designed to provide training to the person and their family members to increase their skills to care for and maintain the ability to live in the home. Support can be provided in the following community living service categories:

  • Community participation
  • Health, safety, and wellness
  • Household management
  • Adaptive skills

These services can be provided to both children and adults in their home or community.

Respite (hourly and daily)

Respite is a break for the caregiver and individual. This service can occur in the family’s home or at a licensed site. Respite care can be delivered in an hourly block of time or set up as an overnight/day. We are licensed to provide respite services to adults and children. We have a licensed community setting where we can provide respite care to adults and children.

Semi-Independent Living Services (SILS)

Semi Independent Living Services is a teaching service that is developed by the client and team to promote and maintain living in the community. The client directs which areas of community living they would like to enhance, and our staff works with them on the skills identified.

24-Hour Emergency Assistance

24-hour emergency assistance is on-call counseling and problem solving and/or immediate response for assistance at a person’s home due to a health or personal emergency.
