Direct Support Activity Professional ($250 Starting Bonus!)

Overview

The Direct Support Activity Professional (DSAP) will assist in providing 1:1 service for individuals in residential services. This position will provide in-home and out-of-home activities and attend appointments as needed. This position will be responsible for planning, implementing and documenting activities and appointments.  This position may work in multiple locations throughout the day/week and be comfortable working with individuals in various settings.  The position should possess excellent communication and customer services skills, both internally and with external customers. This position will be providing direct care while working with the individuals in our service.

New employee starting bonus to be paid after 90 days of employment:

  • Full-time positions: $250.00

Base starting pay:

  • $18.00 – $18.50/hr

Location:

  • Chisholm, MN (traveling)

Available time types:

  • Full-Time

Available shifts options:

  • Days/Afternoons
  • Weekends as needed

Responsibilities

  • Supports the Mission, Vision, and Guiding Principles of NHS Northstar.
  • Supports, cooperates with, and implements specific procedures and programs for safety, confidentiality of data, quality assurance, current rules, agency policies and able to demonstrate mature judgment and able to make programmatic and supervisory decisions.
  • Reporting to the Residential Director but also working with Learning and Development, Program Coordinators and Residential Service Supervisors, the DSAP must have exceptional communication and documentation skills in order to ensure all parties and working to collaborate services and activities.
  • Ability to have a flexible schedule and availability to meet the needs of the department, individuals and employees as determined by agency leadership.
  • Skilled at researching local happenings and creating activity calendars for out-of-home activities.
  • Create and implement arts and craft activities, games, cooking/baking activities and other in-home activities specifically gearing the activities to the individual’s receiving the 1:1 supports.
  • Coordinate, prepare paperwork and attend medical appointments as assigned.
  • Relate to and react to each employee and individuals with unique needs, abilities, and personalities.
  • Communicate and cooperate with all agency departments.
  • Meet on-going requirements to maintain required licensure/certifications and participate in professional training and development as assigned.
  • Is knowledgeable of individual rights and ensures an atmosphere which allows for privacy, dignity, and well-being of all individuals we serve in a safe, secure environment.
  • Coordinates with physicians and other healthcare providers and ensures that individuals receive the proper medical care determined by the treating healthcare professional.
  • Participate in medical appointments with outside agencies.
  • Other duties as assigned.

Qualifications

Experience/Education/Training

  • High School Diploma or GED is preferred.
  • One (1) year of experience working with individuals with disabilities is preferred.
  • Experience coordinating, attending and documenting multiple in-home and out-of-home activities and appointments.

Regulatory Requirements/Licensure

  • Valid Driver’s License.
  • Department of Human Services Background Check.
  • Meet the driving requirements and criteria acceptable to NHS Northstar’s insurance carrier.
